  • 12 year old girl

Problems:

  • 2010-2014: Temporary stoma due to colon cancer.
  • 2019: Ileostomy due to faecal incontinence. Large oval stoma and hernia.

Problems:

    pop-up baseplate.
  • Difficulties in attaching.
  • Regular leaks, pouch often

  • 13 Mar 2019: Acute rectosigmoid resection with end colostomy due to sigmoid perforation (tumour)
  • Very large post-op stoma
  • Patient discharged to nursing home on an adhesive 2-piece system with closed pouch

Nurse review:

  • 29 Mar 2019: Attended hospital –
    hypersensitive severely excoriated skin.
    Evidence of skin stripping and blisters
    requiring adhesive remover to remove baseplate
  • Stoma reduced in size – changed to smaller 2-piece system
  • Blisters treated with powder. Flange extender used around edge
    of baseplate
  • Discharged to nursing home.
    Advised to change baseplate
    3 x per week and closed pouch
    1 x per day. Revisit in 1 week
  • 2 Apr 2019: Painful broken skin with
    blisters. Leakages under baseplate
    at left and right sides. Baseplate
    worn for 4 days. Pouch changed
    once per day

  4. Long term health conditions are likely to affect the person’s ability to manage their own care independently. There will be careful consideration to the specific condition and the individual restrictions when recommending an appropriate product to promote optimum selfcare, management and independence for the ostomate.3

  • 75 year old gentleman with poor mobility who lives alone
  • Diagnosed with Rectal Cancer
  • APER with formation of end colostomy
  • History of chronic illness including Parkinson’s
    Disease, bipolar disorder, hypertension, aortic stenosis

Nurse recommends eakin dot 2-piece:

  • Stoma care nurse assessed individual ability and recommended eakin dot® 2-piece immediately post-op

  3. When independence is not possible, stoma care can be performed by a relative or health care professional.  Many carers worry about
    removing appliances as they feel that this may cause damage to the peristomal
    skin.3

Patient History:

  • 80 year old married gentleman
  • Mr. S was diagnosed with bowel cancer in January
  • Anterior Bowel resection and formation of end ileostomy
  • Post operative complications led to delayed recovery and delayed discharge one month after surgery
  • Mr. S is independent with emptying pouch, but requires full assistance from wife with pouch changes

Nurse review:

  • Initially Mr. S was using a soft convex without a seal
  • Mr. S and wife were advised to change pouch every 2-3 days but often it was closer to 5 days when pouch was changed
  • Leaks and skin irritation occurred

  2. The goal of both nurse and patient is to achieve a
    predictable, reliable wear time.2
  3. Parastomal hernias are one of the most common
    complications reported by ostomates.3 Occurance ranges from 25 to 50 %.4
  4. Postoperative recovery is not always straightforward with some ostomates requiring change of product due to post operative pain and wound management.

Patient History:

  • Mrs A – 73 year old female underwent a large paracolostomy hernia repair
  • This lady was keen to use her previous 2-piece convex system after the repair surgery as it was familiar and comfortable

Problems:

  • Persistent discomfort on the lateral left side
    of the stoma, suggesting the convexity was
    placing pressure on the internal mesh
  • Leakage due to the uneven plane of her
    refashioned stoma

Nurse review:

  • Skin damage around the stoma
  • Mild bruising and fragile skin at 3 o’clock from the convex flange
  • Signs of medical adhesive removal skin injury (MARSI) along the flange edges due to frequent changes of appliance

Nurse recommends eakin dot 2-piece:

  • Less pain and bruising on the lateral mesh side
    Mrs A felt comfortable managing this system at home

Outcome:

  • 2 weeks after using eakin dot® 2-piece with seal under base plate, Mrs A reported the skin as feeling much better
